refactor: changed markdown regexes and related constants

Brief list of changes:
  - removed several headers;
  - added single array of pairs for direct mapping regexes to HTML
    wrapper strings;
  - COMMON_PATTERN splitted into two regexes;
  - wrapper for hrefs is a single constant.
